Most commonly, incorrect duplex receptacles (not dedicated, such as with Microwaves, EV Chargers, etc.), with assumed undersized conductors, as observed by scorched outlets/cover plates/plugs.
Only so many can be attributed to old, out-dated units.
Note: I rarely see this issue in Commercial properties with PTAC units. When I do, it’s usually in 50’s-60’s era structures.

Why were aluminum conductor sizes left off for 15 and 20 amp circuits?

Lighting circuits typically would not be on 20 amp circuits.
