It’s possible that there were some B vents from a water heater and/or gas furnace that came up through the chimney cavity and are now gone. Or are still there and do not extend above the cap. Or, I don’t know.

Just another theory/guess: Mason capping/repairing chimney with mortar; voids in masonry chimney shaft or from previous cap failure, filled with paper or cardboard to temporarily support mortar while drying; paper absorbs moisture; collapses; mortar cap/repair fails approximately 2 minutes after repairman climbs off roof enroute to collect payment for repair job!!!
