Actually two of those apply. I use the digital camera/paper combo & a tape recorder (for roofs, attics & crawl spaces).

At the moment I stick to a pen and notepad; and a wing and a prayer sometimes!
Because I have just started my business my budget is rather slim so I keep it simple, and I do carry a regular 35mm camera for those things worth recording (I actually have the worst picture of a bathroom that most will ever see:a mushroom growing out of the floor beside a leaky shower stall!).
As my budget increases I will be investing in a digital still/video camera to carry along.
