The only “code” I am aware of is that there is a 15" clearance required from the center of the toilet to an adjacent wall. (and it would be measured with the door closed)

IRC 2003 R307.1 Requires 21" clearance in from of a stool to a fixed object. Nothing about a door. My mamma always taught me to close the door.
