Typically around here the type of vent you pictured would be for a closed combustion unit. Meaning air is brought in from outside, burned and then exhausted back to the outside. No air from inside the house is used. The vent would be designed to allow both the intake and exhaust to be in the same unit. This makes a nice clean and simple installation and you only have to have one hole in your wall instead of two. If this is a closed combustion vent as I suspect, the intake will come in though the grills on the side and the exhaust will be going out the grill on the top.
