Gary, are you in a high radon area by any chance ?? I have never inspected one, but I understand there are systems where the water is agitated to release radon gas and is then exhausted.

They are not that new. I installed a water heater in my house that had a power vent in about 1990. The power vent sat on top of the water heater and vented through the side wall (rim joist) with a flexible vinyl hose. They were common on new construction at that time.

I’ve seen this generally used when there is a fuel efficent furnance that is vented out the side wall. There is no vent/chimney to the roof that would provide draft/vent for the water tank. The power vent on the water tank is then directed out the side wall also.
