Waht do you mean by "twist lock outlets in dead front cover"?

If not you can state:
A. Electrical Panel Dead-Front cover secured improperly with flamable tie-wraps?
B. Removed improper fastners at main panel for access. Have licensed electrician secure dead-front as soon as possible.
C. Improper device securing dead-front cover preventing normal access. As no "destructive means" are used in our visual inspection, the panel cover was not removed. Further review, and repair, of the panel is required by licensed electrician prior to close of escrow.

Do you mean that there is a twist-lock receptacle (i.e. NEMA L6-30R) installed in the dead-front cover? This would require either cutting a large mounting hole into the cover, or mounting an outlet box on the cover. Either way, it is wrong. With the recep installed on the cover, the cover is now connected to the box by the wires, and cannot be safely removed. I would state:
"Electrical outlet improperly installed at cover of main electric panel (or sub-panel if it is one). This is hazardous. Recommend further evaluation and correction by a licensed electrician."
