I am pretty sure I answered your question as it pertains to me in the second post. Of course that is just how I do it and you need to make a desicion for yourself on this matter. It will define alot about how you inspect and how your reports look.
Yes, agreed. Focus on the significant issues and those small issues that are indicators of something that may be significant later on. However, the small issues in their totality (that is too many of them) may be seen as significant. This is also true if the small issues are in places that the client does not normally see, then yes you might want to mention them.
