What are the causes of hot water backing up in to the cold water pipe?

Thermal expansion is one. The water in the tank expands when it gets hot, and this pressure could overpower the supply pressure.

No heat trap or heat trap fitting would be another. Heat rises, so the hot water would rise into the supply and outlet lines.

Both of the above would be exacerbated dramatically by a failed dip tube in the water heater.
