Open junction boxes
double taps
Overprotection
Underprotection
illegal / unsafe wire splices (w/o wire nuts)
exposed wiring
unsafe fixtures/outlets/lights
Wrong type wire for application (i.e. Romex used for UF)
Improper/unsafe ground rod connector/conductor
Mixing ground / neutral wiring on bus / overcrowded bus connections
Dont know if this is the correct list, but I can almost always count on at least 4 of the above items to show up on any given house. These are my repeat offenders.

Open J-boxes
Double lugs (neutrals)
Unmarked panels (what the breakers control)
Reversed hots and neutrals at receptacles (Harry the Homeowner strikes again)
3 conductor cables (with ground) used on 2-circuits (with common neutral) and no common trip on the breakers.

Hi Ben:
This item caught my eye and I wanted to go a bit further. The lack of a connector, and use of Type NM cable for these appliances is what I mean.

Y’all missed the
#1 all time repeat offender. White wire not re-identified when used as an ungrounded conductor! Maybe not the most dangerous, but certainly (in my experience) the most prevalent.

There are many others.
How about the disposal and electric water heater, how many electrical items can be found that may be defective?
