Different sized service entrance conductors to service panel - 200 Amp & 125 Amp. Any comments?

I am wondering if maybe the service entrance conductors are the same size…just one of them isn’t where it belongs?
Do you see what I am saying?
Just read what is written on the wire jacket one is 4/0 one is 2/0 the breaker is 150 amp. I think this is okay, just odd.
Yes I realize the conductors going to the bus are different sizes. That is obvious.
It looks like there are two conductors that are the same size though. A red one, and a black one. It looks like the red one is being used as the neutral. You see what I am saying now?

1/0 is feeding the breaker. Should be ok?
You did not mention the disconnect size.
I think it says 150 so the answer is no.
Service 1/0 & 4/0. Disconnect 150, Subpanel 200.
It appears the the 150A breaker is protecting 1/0 conductors leaving the breaker as well.
Those are not protected correctly either.
IMO this needs a rip out and start over. Is there a GEC, if so where’s it terminated?
It’s badly done and certainly should be repaired by qualified persons.

Michael is of course right and since I just got back in the office I checked up on what you have in picture form.1/0 is the smallest but can’t be used for 150 Amp Aluminum.
