Taking the structural test and I need help understanding an image I took recently

I’ve been working with an inspection firm that focuses on multifamily buildings for the past 12 years. Until recently my focus was purely on interior units. I’m now certified through Internachi and working my way through the initial continuing ed courses. Going through structural I was reminded of a complex that we inspected recently, and a very specific issue found within the basement area of the building. Can someone help me understand what’s going on?

I know that I could use a much easier example for the course work, but this particular issue has been stuck in my head since the photo was taken. On the exterior there was signs of erosion on that side of the building, and the Eifs siding showed some signs of cracking (no indication of buckling). When we entered the basement area this stood out like a sore thumb. Heaving and rotation was present.

Major Concern: We observed substantial bowing and cracks in the foundation LOC. This is often the result of excessive soil or frost pressure on the foundation. You will likely need to undertake repairs to protect the building from more serious damage. We recommend you consult a structural engineer who is familiar with foundation repair or a company specializing in foundation repairs to evaluate this condition and provide repair recommendations. Lot drainage and foundation improvements should be addressed to keep water away from the building. We are unable to predict the rate of movement during a one-time inspection.
