If the room volume is sufficient, combustion air can be drawn from inside even with a 90% appliance. In the photo, it is the flue discharging into the house that is the problem and defect.

Zach, IMO there SHOULD be info regarding what you saw and what you described - for the benefit of newer inspectors or perhaps inspectors that are not familiar with the item. For example, if nearly all the homes in your area are electric heat you may not encounter furnaces. The purpose of these forums is for EDUCATION.
