Well brother Dave, you certainly reached way back about 100 years ago for this one. I still run into it though, mostly on old farm houses or in the old, old section of town. The insulators also make the absolute best knife sharpening stones I have ever had.

What do you call a ceiling box when it feeds another box? Is it not a junction box?

My vote no ground(ing) conductor.

The question should have stated “original installation”. There are many homes that have K&T wiring with junction boxes added to them “after the fact”.

I usually find old K&T wiring that has been expanded and improperly spliced using the shiny electrical tape. This splice may appear to be the original wiring, except the tape is not “the old” cloth tape. I recommend that this improper splice be installed into a junction box.
