Are you sure? It looks like a vent termination kit to me that is just missing vanity plate.
The actual vent pipe is like 3 inches, the rest is the thimble/heat shield.

Missing escutcheon ring, the flue should be pitched to allow for condensate disposal (or a condensate line installed) on similar Rinnai units. Check that units specific installation instructions for installation requirements.

Is that a GFCI outlet the WH is plugged into? Rheem does not approve of being serviced by a GFCI or AFCI controlled circuit.
Rheem requires a drip pan under their water heaters when installed in a way that leakage can damage building materials and one is missing.
Sloppy work and I would note it. Also gas lines are not to be used to support anything, including a power cord.
Just as an addendum. The item for sale(see link above) shows two trim plates.
The install pdf shows only the outside trim plate, not the interior plate, which means maybe it’s installed exactly as it should be.
My understanding is that the concentric vent outer sleeve is intake, and the inner pipe is exhaust. Therefore, the clearance from combustibles is likely intrinsic.

Lol, well, I’ve installed boilers with concentric pipes but never a Rheem and never a tankless like that. It didn’t look all that shady to me, but one sorta expects to see a thimble trim plate of some sort on the inside, or at least something to trim out the hole, like fire foam or something.
I’m sure there was a metal escutcheon in the kit, but it never got installed.
