I would guess a speaker or alarm. Did you find a central control unit?
Perhaps a smart home sensor (occupancy sensor).

No. The home owner had no clue either. I thought it might have something to do with radon. I’ve never seen a cover plate like this. It’s a friends house I did a mock inspection on. Might have to go back some time to investigate further.

Any chance there is a heat recover system in the house? Not that I’ve seen a grill like that with such a system. Just a thought.
I’ll have to get the home owner to get up there and take off the cover. It very well could be. Thanks for looking into it.
It’s a speaker cover. Why worry about it? Why even need to know?
If it were on the ceiling of every room, wouldn’t most of us simply climbed a ladder and peaked inside the grill for a closer look? Or looked for the top of the device from an accessible portion of the attic?
Sometimes the best thing to do is look closely at “strange new finds”. Maybe it’s just the curious nature I have.
