Doing a 4 point inspection this looks like a double tapped main breaker I know some breakers this is allowed because the breakers are made for this purpose this appears to be a ITE breaker. Could somebody shed some light on this. Thanks in advance

So that is definitely not okay this breaker is not designed for that correct
And not okay. Besides the triple tap the terminals are corroded and the only possible fix is a replacement.
Multiple issues can come from this scenario. Potential for loose unreliable connections, arching, over heating( Fire hazards ) main wires-strands trimmed to make them fit, may even have copper/aluminum wire mixed on same lug which is not good either, or simply overloading the fuse/breaker. Write up what is there and refer to qualified electrician to repair! Good Catch!
