Adam the cap looked good and original, this was my best shot. These brick were isolated, there were only 4 like this on the whole chimney and it was large and ran from the ground.
Your pic shows cracks and deteriorated/shrunken mortar that may also require re-pointing. I'd refer it to a qualified masonry contractor for eval to prevent further damage.

Almost looks like those bricks are being thrust outwards by concentrated point loads. My guess would be the brick mason inadvertantly allowed a few wooden spacer blocks behind those bricks, and when subject to moisture and freeze-thaw cycles, the blocks are forcing the bricks outward. I’ve seen similar reactions with spacer blocks remaining inside of concrete wall placements–they will spall the concrete right off of the face.
