This is a piece of facia board from the front of a house about 10 ft off the ground. Is this damage from ants, termites, or just wood rot? I don’t see frass, mud tubes, or anything live. There is carpenter ant signs in the basement 2 floors down.

I would just note the wood deterioration and recommend a qualified individual repair/replace.

Look like wood rot. As others stated, note the damage in your report, recommend repairs by a qualified Contractor, and call it a day.
