Electrical cartridge fuse fails to disconnect and fries the panel

Remember, circuit breakers and fuses are mechanical devices, just like car brakes, and they don’t always function as designed. This paper mill panel caught on fire when the fuse allowed the system to overload.

I believe the overload occurred before the fuse failed to break the circuit.

If the damage was related to a loose connection the OCPD might never open which appears to be what happened here.
