Its hard to see clearly but it looks like the stickers on the breakers say “Federal Pacific”
If it is in fact an FP Panel that usually overrides all other issues. It is an automatic consult with licensed electrician.
Not sure about other states but, in Florida it will be hard to find an insurance company that will even insure a house with an FP panel.
I would write up the double tapped lug, and the color of the wire. It needs to be taped white, Blue indicates a 208 volt 3-phase hot leg.
Blue does not mean a hot of a 208 system. Blue can be used for any voltage.

I agree, typically used in 3 phase applications but perfectly acceptable for single phase 120/240 systems as well.
Here Black, Red, Blue is required for 208 and Brown, Orange, yellow for 480. your local codes may have other requirements. in any case you can never use blue for a neutral.
