Is there a standard minimum distance and height combination that a gas appliance “B” vent needs to be from an openable window to prevent exhaust gasses from entering living spaces? A house I inspected today, built in 1993, had a “B” vent terminate level with the bottom edge of upstairs windows, 5 feet from an openable hallway window and 6 feet from an openable bedroom window. Seems like an obvious risk of CO poisoning to me.

The 3-2-10 rule does not apply to B vents with B vent caps. With unapproved caps or vents, yes, 10 feet applies.
8 feet from the wall or 2 feet higher than it for listed B vent sizes 12" and smaller.
Don't know of anything addressing windows, maybe because it is already required to be 8 feet from any wall ...
