Main breaker height

Can someone give me the reference on how high above the “surface” can the main breaker be., I know it has to be “readily accessible” but I can’t find the ref to how high. I thought it was 6’8", but maybe a brain fault has hit me. I can’t find the code I saw once (don’t freak, the code is for me).
thanks
