Has anyone seen a roof dryer vent with a quarter size hole in the top of it? Very concerning to me
No… tha’t unusual.
I seem to recall seeing that a few times over the years, always assumed some dryer vent cleaning outfit drilled a hole to facilitate their process, but wrote them up anyway.

I would not worry about the Loony or Silver dollar sized opening. I suspect it is to prevent condensation. As well, I suspect the manufacturer took care of that point of entry for vermin, rodents and birds with a screen and an engineered screen with channels to disperse bulk water from the opening.
Relate to the average amount of condensation created drying clothes in a clothes dryer. Glasgow University study found that most loads released between 2 to 2.5 litres of moisture.
Personally, I never liked roof top clothes dryer vents because, what goes up must come down. Its called gravity.
Those that see drying clothes in a baby’s room or the basement while inspecting homes any time. Look for signs of mildew and mold. A single load of clothes drying indoors adds roughly 4 US pints of water to the inside of your home.
It’s a roof leak… into the interior of the duct, but still a roof leak.
Are you implying that is a drone image and not an “on the roof or on the ladder image”? Pity if it’s a drone shot. In that case, what else was missed or misinterpreted by not inspecting the roof from a ladder at the roof edge?

The angle kinda looks like a drone shot, but only the OP can answer that. Better image quality would help. It’s the little light spot on the right side of the black circle that makes my mind think it’s a plug, but could be an optical illusion.
I wonder if the flap is restricted by lint and doesn’t close completely.
See it all the time, or a screen installed, lint catcher, painted shut, or no damper at all!

What I think.
The hole is there for a purpose.
1: Possibly, the hole was made to clean the vent then plugged.
2: If engineered that way, with a hole, the manufacture installed precautions to allow bulk water to exit prior being dispersed down the vent.
