I would say a loose lug. If I am not mistaken you can see in the pic you provided they lug is melted.

Did you mention that the neutral wire should not be running across the two service entrance cables?

I agree with your assessment, except for the concern with regard to mixed neutrals and grounding conductors. As this isn't a sub-panel, I see one common buss bar all the time in older panels.

PS?: You're right, the guy probably crossed his neutral and main and is still suffering from welder's flash as we speak! BAM!!!
