ends out about 4 feet for balconies. Any idea how or what type of concrete floor system this is.

Looks like some type of precast concrete panel.
I agree about it likely being precast. Are you concerned about the cracks? It looks like some concrete missing maybe due to rusty re-bar? If the holes for the plumbing were bored on site it may have compromised the manufacturers responsibility. These are manufactured under strict guidance from engineers and aren’t supposed to be modified. Consider having a suggestion for an engineer review. Balconies falling off have become a huge liability issue in north america and in the event of a civil suit anyone who looked at it is going to get sucked into the vortex.
After looking in more detail it’s precast. Here is a 3d model with optional voids. Some of these required a concrete overlay, if that’s the case the top of the precast panel would be have a rough texture to help bond the top layer. The individual panels would likely have a tongue & groove interlocking edge and the joints would be beveled to provide for caulking. I have inspected many bridges with precast panels with and without the concrete overlay. All suffered from water/salt damage at the joints, with spalling like in your photo from rust expansion.
Yes the cracks and spalling is a concern. Indicates the surface and/or cracks are not sealed properly. Also looks like light weight concrete was used, which is common.
