The pic is a little small - but improper size and type vent piping and questionable heat loop on hot water line are obvious. is the T&P valve coming from the top or is it on the side and extends above the heater?

Can’t tell from the pic. Is there any source for combustion air?

I don't understand this. Could you explain?

It looks to me like an "A" vent pipe is connected to the draft hood and then connected to a "B" vent.

Don’t have the code book in fron of me but believe the 2003 IECC requires water heaters with vertical risers must have integral heat traps or heat loop on the inlet and outlet of the water heater. Believe that is section504.7?

A lot of dielectric unions have heat traps built in.

Anybody have a good illustration or picture of the proper set up?

Judging from the obvious age of the system, no current code would have been in force during installation and as long as single wall vent pipe did not penetrate a wall, floor or ceiling it would have been perfectly fine to use it. (at least here in Ga) I see nothing wrong with this installation. I would note the obvious leaking on the hot side though.
