For #1 on the mitigation form, if the unit is not 2001 or 94-96 for example a 1998 - 2000 build, then is the unknown or does not meet the requirements of A or B box selected?
If the permit application is on or after 09-01-1994 or, as you say 1998-2000 build, select B & fill in the year date if you do not have the permit date.
You’re in the HVHZ so use the SFBC for reference, not the state FBC.

No, just go with the year built.
If the year built is 1994-1995-1996, then you’ll need the actual application date.
Otherwise use the year date you have.

Thank you again, Im going to ask another question if you dont mind. If a product approval number for this roof for example is un known then the roof still falls under category B or C? Reason i ask is because its built fbc certainly, but i dont have a product number confirming product approval for the B selection, how does that part work? Im not trying to guess or make up anything, i just want to report correctly
Number 1 select A, then fill in 2008 for year built.
Number 2 first select type of covering (singles, cement tile, etc.) then fill in the permit application date. Then select A - all roof coverings listed meet the FBC.
That’s it you’re done.
You only need to choose one, yes you can do more if you have the info.
