Your 120v outlets are labeled x and y when they should always be x and w (120v uses a neutral or grounded conductor).

The chart doesn’t show 120/250 4 wire like a newer dryer or stove would have.

I would use ungrounded instead of positive conductors. Positive and negative relate to DC circuits.
