Your 120v outlets are labeled x and y when they should always be x and w (120v uses a neutral or grounded conductor).
I agree.
According to the labels, the 15 and 20amp 120V receps are getting 240V and the 240V 30A recep is getting 120V.
“Positive” implies a DC circuit.(Maybe I inferred that).
I think a more appropriate labeling would be:
G-ground
N- neutral
A- A phase
B- B phase
C- C phase
