Yes, that is a low voltage transformer of some sort. More likely for the doorbell. And NO, they do not go inside junction boxes. That installation is typical. Most of the time a doorbell transformer will be attached the the side of the SE panel or somwhere on the wall within that area.
As for the wiring, You have a bunch of spaghetti nm romex wiring mixed in with telephone wiring throughout the attic. I just note in the report that exposed wiring (in attic) is not running through floor joists and may be pinched if walked on or if flooring is installed.
Half my attics here in Massachusetts resemble the same manner.
