If the cord is concealed, goes through a wall or floor etc it is a clear violation.
There are times a cord can be connected to the building wiring but it will need the proper connectors and need to be out in the open.
An example is the pendant receptacle you see serving a fryers and warmers in a fast food joint. Those are usually on wheels and they can be moved for cleaning. They need the receptacle low enough to disconnect without climbing on the hot fryer.
