Accessible junction boxes, in the attic or other place, are acceptable if installed properly. 
Do you mean “accessible” as in no cover plate?

Yes, but I suggest that you try to refrain from using the term “acceptable” when writing reports. What is acceptable or unacceptable is up to the buyer (or AHJ). Try to stick to terms such as defective, deficient, improper…
