Sinkhole Inspection Form

Good job on the form Eric. Scanning over those inquiries on the second page I found that about 85% of the homes I inspect and have inspected would have a yes on most questions, yet we are not in a sinkhole prone area of the State. Cracks in concrete around here are pretty much the norm due to poor soil base prep. They show up with the concrete the day after the forms have been set and pour right on top of dry soil, which of course sucks every drop of moisture making the concrete dry out. Does not cure properly especially in the heat of the summer and shrinkage cracks begin to form before the trucks are out sight.

Same for depressions in the earth / soil. A lot of the contractors grind out stumps leaving the chips in the holes, cover it with red clay and a few years go by, the wood chips decay leaving the home owners with a nice deep depression and a bumper crop of mushrooms.

That is Citizens form which I made a few modifications to, so that it can be made into a fillable pdf.
The original one Ted sent me was a scanned version.
I have a Word version where I cleaned everything up to make that pdf.
