I write " new policy " for the answer to that question unless someone tells me who it is for. Never had a problem yet
I too write “Pending” on the form and have yet to have it questioned.

It light of the known different Insurance requirements, would it not be a good idea to take photos of each of the items requested? That way you have the photos and information should it be requested later.
If I look at it, I take a picture of it.
I also always leave the insurance company info blank.
I think what Richard Hyland is trying to say is: What do you inspect when the insurer is unknown. This is likely in response to those that have said that "if the insurer doesn’t ask for the brand of the breaker panel I don’t provide it.’ He wants to know what they do when the insurer is unknown. Do they provide the panel brand or not.
My personal practice over the last several years of performing 4-points, right or wrong, has been to develop my own inspection form based the composite of insurance company requirements. When ever I run into an insurance company whom I haven’t performed an inspection before I request a copy of their 4-point inspection requirements either from the agent or the company directly. Haven’t had one yet say inspector: inspect whatever you think and we will run with it. They always know what they want. When it is Citizens I provide my report and the NACHI report approved by Citizens a little more work but my pictures are in my report and it mabe take ten more minutes to fill out the Citizens report.
